Entry Form

In order to avoid saving, calculation or display errors, follow these steps before filling in the entry form:

1.  save the empty Entry Form on your local hard drive (left click on the PDF icon, save target as)
2.  open a recent version of Acrobat Reader (XI, DC) or Acrobat Pro
3.  open the Entry Form from the menu of the Acrobat Reader (File – Open)
